Older adults frequently present to nursing home care with long and burdensome medication regimens. Poly-pharmacy can contributes to increased pill burden, increased medication-related costs, and potential adverse effects. This session will use a case-based approach to review indication and evidence of benefit for several commonly prescribed medications including proton pump inhibitors, multivitamin supplements and probiotics with the goal of promoting effective and safe prescribing.

This session will describe a variety of collaborative practice models that can be utilized by advanced practice providers (nurse practitioners and physician associates) and physicians across different PALTC settings (skilled nursing facilities, nursing home, assisted living, and homecare). Participants will learn how advanced practice providers can be actively involved in meeting PALTC regulatory requirements, expand billing opportunities, and address quality improvement initiatives in PALTC collaborative practices.
This session will present an overview of the literature regarding falls with head trauma. The use of anticoagulation with residents who fall with definite/potential head trauma will be addressed. Staff neurologic assessments after falls will be discussed along with liability concerns.
This session will review current initiatives in Geriatric Deprescribing with particular focus on the LTC clinical setting. A general update of deprescribing practices will be presented. The most recent update of the Beers Criteria will be reviewed highlighting applications to the LTC population. The international START and STOPP criteria from the 2023 update will be reviewed with a focus on interventions most appropriate to the LTC practice setting.
